As a member of DPC, you’ll have access to the following:

• Through and relaxed visits. Part of the relationship you’ll build with your doctor is based on your understanding of your care... this is "personal" healthcare.

• More availability for same-day and urgent appointments.

• You’ll be able to reach our team via phone, e-mail, and text via the Elation Patient Passport.

• An annual physical - This exam will be fitted to your personal medical needs. It’s all about you and your specific wellness goals.

• Diagnostic and procedural benefits at no extra costs.

           EKG, Spirometry, Breathing Treatments, Cryotherapy.
